Asian Pacific Health Care Venture (APHCV)’s mission is to be the medical home for their patients by providing personalized, quality health care services in a culturally accessible manner.
They began providing their unique brand of health care through a primary care program subcontracted to various clinics in our community. They also collaborated with other local health organizations to begin providing health education and awareness on topics including youth empowerment, HIV/AIDS awareness, tobacco use prevention among teens and adults, tuberculosis, and hepatitis.
They continue to improve their care by constantly seeking out the best possible health care techniques and technologies, launching an Electronic Health Record (EHR) system in the summer of 2012. They are working towards Patient Centered Medical Home certification so that patients are confident that their providers and health care team have the tools and resources to deliver the best care possible and patients have a health care home for years to come.
